Carers of Older people (65+) In Norwich and South Norfolk,

An exciting new pilot service in the Norwich and South Norfolk CCG areas delivered by the Carers Agency Partnership for carers of older people (65+). We have recruited two Community Care Coaches, experienced care support workers, who have undertaken an 18 month care coaching course (harm free care) with the Norfolk and Suffolk Dementia Alliance. Our coaches can offer personalised one to one coaching to carers of older people in their own homes on a range of very practical topics designed to equip and empower them to continue caring with confidence and promote their wellbeing. We’ve designed an Education and Coaching programme which includes:



1. HARM FREE CARE

· How to avoid a Urinary Tract Infection (UTI)

· How to spot the signs of a Urinary Tract Infection (UTI)

· How to reduce falls risks

· How to avoid and/or minimise risk of pressure ulcers

· How to avoid and/or minimise risk of Chest Infections

2. CARE OF PHYSICAL HEALTH

· Supporting the independence of the cared-for (including risk assessing and avoiding inappropriate moving and handling)

· Back care for the Carer

3. NUTRITION & HYDRADTION

· How to encourage individuals to eat when they have no appetite

· How to encourage individuals to drink their minimum daily fluid intake

· Meal Planning - menus and ideas on how to produce tasty healthy food on a limited budget or with tight time

· How to manage weight and prevent obesity

4. AVOIDING ESCALATORY TRANSFER of CARE (PREVENTING HOSPITAL ADMISSIONS OR RESIDENTIAL CARE)

· How to develop Carer Emergency Plans

· Where to get support and knowing when to ask for help– accessing primary and community health services and/or social care

5. CARER RESILIENCE

· Strategies for coping and developing emotional resilience

· Carers Wellbeing



Carers will receive a coaching information pack with the relevant support information and will be referred into the Carers Agency Partnership’s menu of services (advice line, individual support, short breaks and grants & funding and the online information hub) and other support services where required.



HOW TO REFER: call our ADVICE LINE on 0808 808 9876 to make a referral – one of our coaches will contact the carer to make appointment to visit them at home. Please could I ask for your support in referring carers you work with or know or giving them this information so they can self-refer to the service. We would really appreciate your support.
