A care home in Leyland has been placed in special measures after all of its services were rated as "inadequate" by inspectors.
The Care Quality Commission (CQC) made an unannounced visit to Hazel House care home on 14 and 15 July.
A CQC report said residents faced "significant risk" of harm and neglect and their "dignity was compromised".
It added that staff were insufficiently trained and did not respond to the needs of residents quickly enough.
The report found a number of breaches to regulations in relation to safety, staffing and the administration of medicines.
Unresponsive staff
It revealed one person had not had a bath or shower for three months. Three others went without a bath or shower for two months.
One person who requested to visit the toilet was asked by a staff member: "Are you sure you really want to go?" it added.
Some relatives told inspectors they often found residents wearing other people's clothes and some had been left in soiled clothing.
Others gave positive comments about the care provided.
Parts of the building were not safe and staff were not properly trained to deal with mental health and dementia cases, according to the CQC.
